PDE Requirements
PDE has indentified specific requirements that a public school teacher must meet to be a Cooperating Teacher for Student Teachers in the final field experience.
Cooperating Teacher Survey- Please take some time and fill out the following survey. This information will provide valuable feedback about how our teacher candidates are performing inside your classroom.
The following information will help you work with students at any of the four stages of PDE-required field experience. If you have any questions or concerns, please contact the Office of Field Services at 814-393-2144.

This manual explains which course include field experience requirements, which stage(s) of field experience are within the course, and what the student must do when visiting the public school classroom.

This handbook contains the role and responsibilities of the Cooperating Teacher.

Student Teacher Attendance and Grade Form (PDF):
This is the form to be used for the final grade report at the end of the student teacher's placement.
Teacher Candidate Manual for Student Teacher
Lesson Plan Format for PreK-4, old Elementary Education, and Middle Level Education
This format is to be used by the student teacher for the first two lessons developed for the placement. After this point, if the Cooperating Teacher wants the form revised to one that is more compatible for the school, this can be done.
Lesson Plan Format for Secondary Education and K-12 Programs
This format is to be used by the student teacher for the first two lessons developed for the placement. After this point, if the Cooperating Teacher wants the form revised to one that is more compatible for the school, this can be done.
Student Teaching Performance Profile (STPP):
These evaluation forms, one for each major, are used to define the student teacher's progress throughout the placement. It is often helpful to print one out and use it periodically to show the student teacher strengths and weaknesses and to document progress or the lack of progress if necessary. The document can be printed (below) and completed by hand as needed:
