Distinguished Award Nomination Form
This is your opportunity to nominate a deserving candidate for a Distinguished Award.
There are four award categories: Distinguished Alumni (up to two awards presented each year); Distinguished Young Alumni; Distinguished Service; Distinguished Faculty; and Distinguished Venango College Alumni. Descriptions and criteria for each of the categories are available here.
A. Please complete a form for each nominee.
B. Attach a one page written statement explaining why you believe the nominee should
be recognized and include any supportive information suggested in the descriptions
and criteria for each category.
Nominations are accepted at any time. However, nominations must be received by October 31 to be considered for awarding in March of the following year. Nominations are considered for up to three consecutive years.
