PDE has identified specific requirements that a public school teacher must have 3 years certified teaching experience to be a Cooperating Teacher for Teacher Candidates in the final field experience.
Cooperating Teacher Survey
Please take some time and fill out the cooperating teacher survey included in a mailing you will receive from the Office of Field Services. This information will provide valuable feedback about how our teacher candidates are performing inside your classroom.

PDE Field Experience Requirements Course Requirements by Certification Major:
This manual explains which course include field experience requirements, which stage(s) of field experience are within the course, and what the teacher candidates must do when visiting the public school classroom.
Cooperating Teacher Handbook:
This handbook contains the role and responsibilities of the Cooperating Teacher.
Dispositions Form & Rubric:
This rubric and form is to be used by the Cooperating Teacher to assess the teacher candidate's professional disposition during the placement(s).
This is the form to be used to report any missed days at the end of the teacher candidate's placement.

This format is to be used by the teacher candidate for the first two lessons developed for the placement. After this point, if the Cooperating Teacher wants the form revised to one that is more compatible for the school, this can be done
Mid & Secondary Lesson Plan Format
This format is to be used by the teacher candidate for the first two lessons developed for the placement. After this point, if the Cooperating Teacher wants the form revised to one that is more compatible for the school, this can be done.
Student Teaching Performance Profile (STPP):
These evaluation forms, one for each major, are used to define the teacher candidate's progress throughout the placement. It is often helpful to print one out and use it periodically to show the teacher candidate's strengths and weaknesses and to document progress or the lack of progress if necessary. The document can be printed (below) and completed by hand as needed:

Cooperating teachers of BLOCK students can receive 10 hours of Act 48 for each student they supervise. Cooperating teachers must fill out a Pre- and Post- STPP for each student during the placement. Coops must also complete and Act 48 Application Form. All documentation should be turned into the Office of Field Services at the end of the student placement.
