Informed Consent Notice

Please Read the Following Carefully:

ABOUT THE SURVEY:  You are invited to complete the LibQUAL+™ survey. Participation is voluntary. You may withdraw your participation at any time without penalty and you may decline to respond to any survey item. The survey takes approximately 10 minutes to complete.

This survey will:

• Help Clarion University Libraries better understand how the Clarion University community rates library services.

• Allow us to compare Clarion University results against other libraries participating in the survey to determine best practices. 

• Let us know where we can concentrate service improvements for Clarion University users.

RESTRICTIONS:  You must be 18 years old or order to participate in the survey.  Clarion University students, staff and faculty ONLY may participate in the survey.

CONFIDENTIALITY:  If you choose to participate, your responses will be held in confidence.  No identifying links between responses and the individual respond will be retained.  Combined data only will be reported.
